About the Role
This RN Care Manager position is located in the Primary Care Service Line.
What You'll Do
- →This nurse provides gender-specific direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department.
- →Duties include, but are not limited to: 1.
- →Leads and organizes delivery of care to ensure continuity of care and peer accountability for practice in alignment with the ANA Standards of Nursing Practice. 2.
- →Applies advanced clinical knowledge/judgment to lead staff involvement in planning, decision-making, and evaluating outcomes beyond the immediate practice setting. 3.
- →Applies nursing expertise to lead improvement, with demonstrated outcomes, beyond the immediate practice setting. 4.
- →Demonstrates ability to be self-directed in using the nursing process to improve the outcomes of care at the program/service level. 5.
- →Applies collaborative team approach in identifying, analyzing, resolving problems, and sustaining improvements. 6.
- →Applies problem solving skills beyond the unit level, leading to improved care/outcomes. 7.
- →Leads and directs others in organizing the delivery of care to assure continuity of care and peer accountability for practice. 8.
- →Evaluates program/process or medical center policies/procedures and reports effectiveness or provides leadership, analyzing and addressing ethical legal issues that impact patients and staff and the medical center. 9.
- →Demonstrates knowledge and compliance of Ethical Resolution Process. 10.
- →Assumes responsibility and demonstrates accountability for all professional nursing actions. 11.
- →Provides considerate care, respecting patients'/staffs' personal values and belief system in provision of care. 12.
- →Supports and enhances patient self-determination. 13.
Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.
- ✓Selected applicants will be required to complete an online onboarding process.
- ✓Acceptable form(s) of identification will be required to complete pre-employment requirements (https://www.uscis.gov/i-9-central/form-i-9-acceptable-documents).
- ✓Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Physical aspects associated with work required of this assignment are typical for the occupation and would generally not require a pre-placement examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
- ✓You may be required to serve a probationary period.
