About the Role
About the Position: The Nurse (Administration) reviews and conducts clinical management of the Referral Management process for the Medical Treatment Facility (MTF). This is a Direct Hire Solicitation.
What You'll Do
- →Manage a clinically based program designed to provide high quality care coordination to effect seamless health care delivery.
- →Identify problems with health care access to recommend improvements.
- →Oversee referral, admission, transfer, or continued stay in the hospital to ensure appropriate service is provided.
- →Secure referral or alternative options for patients to resolve delays in care coordination.
- →Analyze data concerning referral management and care coordination for higher reporting.
