About the Role
About the Position: Maintains responsibility for program management and implementation of the Defense Health Agency System of Care (DHA MHSOC), Mental Health Caring Communication / Case Management (MHCCM) Program operations. This is a Direct Hire Solicitation.
What You'll Do
- →Serve as expert mental health case manager and member of multidisciplinary team of medical and professionals to identify high risk and high-cost diagnosis.
- →Perform difficult and complex assessments; conduct interviews; evaluate and analyze health care plan and resources; and plan, coordinate, and monitor the delivery of services.
- →Develop, implement, communicate, and coordinate individualized empirically informed patient treatment plans that address all diagnosed conditions and health concerns.
- →Observe and evaluate patient behavior and responses, to update treatment plans in consultation with the healthcare team.
- →Chair and coordinate the weekly At-Risk/Multi-Disciplinary Treatment Planning (ARCT/MDTP) meetings to ensure treatment plans are synchronized among medical treatment providers.
- →Collect and analyze data to identify trends and patterns and make recommendations for program enhancements.
- →Manage program resources effectively, to ensure that they are used in a fiscally responsible and ethical manner.
- →Develop and recommend strategies to improve healthcare access, reduce administrative barriers for beneficiaries, and enhance the efficiency of the Medical Treatment Facility (MTF).
