This position is located in the Mental Health Service line at the VA Portland Health Care System (VAPORHCS) within the Behavioral Health Interdisciplinary Program (BHIP).
What You'll Do
- →The duties of this position include, but are not limited to: Develops an assessment of the Veteran in collaboration with the interdisciplinary treatment team, the Veteran, family members and significant others.
- →The goal of the assessment is to highlight the Veteran's strengths, limitations, and internal/external supports and service needs in order to optimize the Veteran's functional status.
- →Provides goal planning, evaluation, and intervention with serious and complicated, highly diverse cases involving psychiatric illness.
- →Develops and/or contributes to the development of the mental health treatment plan and the setting of achievable treatment goals with the Veteran/family and with the interdisciplinary treatment team.
- →Provides consultation and makes recommendations to the interdisciplinary team on course of treatment.
- →Maintains an acceptable level of productivity per guidelines which may include special assignments.
- →Collaborates with other service providers in reassessing the Veteran's needs.
- →Responsible for educating the Veteran and/or caregiver of the available services and assisting them with appropriate referrals based on the Veteran's preference or that of his surrogate decision-maker related to mental health issues.
- →Responsible for using current social work resources and community social service programs and enhancing the content to the benefit of Veterans.

Requirements
