The Substance Use Disorder (SUD) Treatment Program assumes responsibility for the provision of substance use disorder services to veterans who are admitted for outpatient SUD treatment.
What You'll Do
- →Duties of this position are, but not limited to: Conduct psychosocial assessments to determine needs of veterans and family members.
- →Conduct intake substance use disorder assessments for patients to determine appropriate VA program needs.
- →Active participant in the treatment planning process with other disciplines.
- →Participate in transition/discharge planning with other disciplines and will be responsible for ensuring that transitions plans are executed in a manner that is timely and appropriate.
- →Provide individual, family and group counseling as appropriate.
- →Individual and group services will be focused on substance use disorder treatment.
- →Provide consultation/education to veterans and families on community resources, advance directives and VA benefits.
- →Act as a consultant to other team/staff members.
- →Provide consultation/education to students of all disciplines on clinical practice and VA policy and procedure.
- →Provide case management to veterans and families, as needed, throughout the continuum of care.
- →Provide a full range of social services within commonly accepted standards of their profession, which includes case work, individual, family and group counseling, discharge planning, case management and other services as appropriate.
- →Provide wellness/prevention education and facilitate patient/family support groups as appropriate.
- →Coordinate treatment and communicate with the care lines to facilitate continuity of care for veterans.
- →Other duties as assigned Work Schedule: Monday-Friday; 8:00am-4:30pm, and 1 evening a week 10:00am-6:30pm Telework: Ad-hoc may be available Virtual: This is not a virtual position.
Requirements
- ✓You must be a U.S.
- ✓Citizen to apply for this job.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Must be proficient in written and spoken English.
- ✓Subject to background/security investigation.
