About the Role

The Patient Aligned Care Team (PACT) LPN is a full-time position located at The Louis A. Johnson VA Medical Center/Patient Care Services . The LPN functions as a member of the health care team by performing assigned duties in a manner that promotes quality health care, positive customer service and the maintenance of an environment committed to serving the Veteran.

What You'll Do

  • The primary purpose of the Licensed Practical/License Vocational Nurse (LPN/LVN) is to perform assigned duties in a manner that promotes quality health care, positive customer service and the maintenance of an environment committed to serving the Veteran.
  • Performs within this functional statement under frequent guidance of the Nurse Manager or designee.
  • Assigned tasks require basic technical competence and skill. the Licensed Practical/License Vocational Nurse (LPN/LVN) is responsible and accountable for individual nursing practice.
  • The responsible duties include but not limited to the following: The incumbent is responsible for delivering clinical and clerical support to the PACT healthcare team in accordance with established policies, procedures, and protocols.
  • The PACT LPN works collaboratively with the healthcare team to provide patient centered care to veterans, ensuring efficient and effective clinical operations.
  • Held accountable for delivering direct patient care from adults through geriatric Veterans for both acute and chronic diseases in an outpatient setting.
  • Implement basic and advanced LPN procedures as defined by policies and procedures.
  • Completion of pertinent clinical reminders and preventative health screenings.
  • Prepare and maintain exam/treatment rooms, including organizing supplies and equipment necessary for patient care.
  • Assist with minor clinical procedures and ensure safety and comfort of patients during procedures.
  • Providing patient and family health education or instruction, to include self-management, prevention and wellness.
  • Document patient care activities accurately and in a timely manner using the electronic health record (EHR) system.
  • Participate in team huddles and case conferences, providing input on patient care plans and follow-up strategies.
  • Assist with patient scheduling and appointment follow-ups to optimize clinic workflow.

  • Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
