About the Role

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

  • The LPN is responsible for observing, recording, and reporting changes in the patient's condition, as well as assisting in the development and revision of patient driven care plan.
  • The LPN is a member of the expanded team and demonstrates active collaboration and communication with the core team (Patient, Primary Care Provider, RN Care Manager and Clerical Associate) and the expanded team (family/caregiver, internal and community-based services involved in providing care to the patient).
  • Major Duties include but are not limited to the following: Possesses the knowledge and skills to carry out more specialized patient care activities, including the use of veteran care data to initiate active contributions to the overall health plan and observing present status to implemented interventions to improve, restore or maintain the optimal function of the veteran.
  • Possesses knowledge of program regulations to carry out objectives of the Nursing program.
  • Contributes to the development of the plan of care for assigned veterans outlining program modalities and updates per policy and as changes occur.
  • Promotes and demonstrates nursing concepts in the Medical Center.
  • Works in collaboration with RN and/or HCP.
  • Prepares and administers prescribed medications and performs treatments according to established policies/procedures.
  • Takes appropriate actions based upon communications, observations, veteran care reports, and provider/staff discussion.
  • Assists other staff to prioritize the overall veteran care needs and adjusts plan of care as needed.
  • Accurately and consistently reports and records significant veteran information received from other team members and can analyze and implement modifications based on these reports.
  • Is adept at encouraging and communicating with challenging psychological and physical behaviors to promote optimal functioning.
  • Demonstrates the ability to work independently without the need for readily available supervision and communicates effectively with veteran, families, caregivers, professionals, and other supportive staff.
