About the Role
The Intermittent Licensed Vocational Nurse (LVN) provides patient-centered, team-based nursing care to Veterans at the Santa Rosa South VA Outpatient Clinic. The LVN delivers high-quality, coordinated care focused on prevention, chronic disease management, health promotion, and continuity of care. The LVN serves as a key member of the interdisciplinary PACT team, supporting timely access to care, effective communication, and population health management.
What You'll Do
- →**This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until July 24, 2026.
- →The initial cut-off date for referral of eligible applications will be June 9, 2026 with subsequent cut-off dates every 2 weeks.
- →Eligible applications received after that date will be accepted on an ongoing basis and qualified candidates will be considered as vacancies become available.
- →VA Careers - Licensed Practical Nurse: https://youtube.com/embed/Ae85IP1Oiz4 Duties and responsibilities of the LVN/LPN include: Perform vital signs, pain screening, health factors, and clinical reminders.
- →Conduct patient intake, rooming, and visit preparation.
- →Administer medications and immunizations per VA policy and scope of practice.
- →Perform wound care, injections, EKGs, point-of-care testing, and specimen collection.
- →Monitor and document chronic disease indicators (e.g., diabetes, hypertension).
- →Recognize and report changes in patient condition to RN/provider.
- →Assist with telehealth visits and remote monitoring when assigned.
- →Support care coordination by assisting with referrals, follow-ups, and consult tracking.
- →Participate in PACT huddles, team planning, and daily workflow management.
- →Assist with population health management activities and panel management.
- →Support continuity of care across primary care, specialty, and community care.
Requirements
