CNH Social Worker Coordinator

About the Role
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
What You'll Do
- →The Contract Nursing Home (CNH) Social Worker serves as a coordinator and primary clinician for a collaborative clinical and residential program.
- →The CNH team provides oversight for Veterans placed in the contract nursing homes in Central or Western MA.
- →The Social Worker and the RN on the team oversee the medical care provided by the CNH.
- →The social worker assists with the daily operation of this collaborative program.
- →Duties for this position include but not limited to: Clinically and administratively screens all referred Veterans for admission to the program/CNH.
- →Provides clinical/administrative intervention and case management to Veterans in the program.
- →Establishes a collaborative treatment plan with the Veteran, the CNH team and RN.
- →Coordinates and functions as the liaison for these CNH beds among team members, CNH staff and families.
- →Coordinates/oversees treatment with the community residential treatment partner Conducts ongoing assessments on a monthly basis in person and as necessary by phone.
- →Other duties as assigned.

- ✓Must be proficient in written and spoken English.
- ✓Subject to background/security investigation.

- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
