About the Role

The Clinical Pharmacist serves as a representative of the Pharmacy Service in a variety of functions throughout the Northern Arizona VA Healthcare System. The practice of Pharmacy consists of direct and indirect patient care, drug therapy monitoring and patient education. The incumbent is responsible for providing pharmaceutical care to optimize patient response and resource utilization for professional staff.

What You'll Do

  • The Clinical Pharmacist has the full time responsibility for providing pharmaceutical care to optimize patient response and resource utilization for professional staff.
  • The incumbent carries out the following duties and responsibilities but is not limited to: Monitors therapy on an ongoing basis to assess therapeutic outcomes, adverse effects and compliance.
  • Analyzes laboratory data to rule out drug interferences.
  • Adjusts or enters medications by discontinuing and/or generating new/renewed prescriptions for items other than Controlled Substances with the verbal or written approval of a physical or authorized prescriber.
  • Takes appropriate measures to ensure that therapy is continued for those patients who are followed at NAVAHCS, its affiliated clinics and at other VA facilities.
  • Interviews patients, as needed, to determine medication history for potential drug interactions, therapeutic duplications, efficacy of current medication regimens, compliance patterns and/or drug reactions and act to resolve identified problems.
  • Reviews available medication profiles for appropriateness, contraindications to drug use, side effects, relevant laboratory data, evidence of drug use/misuse/abuse, compliance, patient education needs, therapeutic duplication, poly-pharmacy, allergies, adverse drug reactions, drug-drug interactions, drug-food interactions and cost minimization.
  • Recommends therapeutic alternatives based upon the patient care assessment and desired outcome, drug safety, efficacy, Pharmacy Benefits Management (PBM) initiatives, formulary management and cost to optimize drug product selection.
  • Provides drug information to the Medical Center staff concerning proper drug selection, optimal dosage and route of administration, recognition and management of drug interactions and reactions, and proper handing and storage of medications.
  • Responsible for the safe and appropriate distribution or dispensing of medications with the Pharmacy Service ward stock, automated dispensing units and ambulatory care medication management systems.
  • Responsible for the interpretation and verification of inpatient and outpatient medication orders and a working knowledge of the medical facility treatment areas and USP 797/800 practices.
  • Conducts medication reconciliation, documents allergy and adverse drug reaction information, and evaluates medication orders for appropriateness.
  • Serves as a liaison between the patient care areas and the centralized pharmacy distribution area to identify and correct problems.
  • Responsible for evaluating, adjusting and monitoring of medications according to current practice guidelines.
