About the Role
The Cincinnati VA Medical Center is looking for an Inpatient Clinical Pharmacist professional with knowledge and proficiency in pharmacy practice including pharmaceutics, pharmacokinetics, pharmacodynamics and pharmacotherapeutics. The employee will be responsible for analyzing real and potential drug-related medical problems and implementing corrective action to ensure that patients receive optimal drug therapy.
What You'll Do
- →Pharmacy Operations/Processing: Workflow should be organized to complete the workload as accurately, timely, and efficiently as possible.
- →Prescription/order processing will be performed in accordance with The Joint Commission accreditation standards, ASHP pharmacy practice standards and VHA requirements.
- →Reviews all medication orders for appropriateness, drug selection, dosage, route of administration and the amount, prior to dispensing.
- →Ensures all medication orders are entered into the electronic patient medical record.
- →Provides medication counseling to patients Quality and Patient Safety: Performs a thorough review of all prescriptions/orders processed, checked, and dispensed, uses clinical judgement when making recommendations.
- →Processes all orders in -drug, drug-nutrient, drug-laboratory and drug-disease state interactions.
- →Assessment for clinical relevance should be performed and dealt with appropriately.
- →Performs continuous evaluation of prescribed medications to assure optimal drug therapy.
- →Assesses drug safety and efficacy, including evaluation of physical symptoms.
- →Reports adverse drug events (ADE), near misses, and medication errors in alignment with VA ADERS, JPSR, and any other pertinent reporting programs.
- →Provides patient-specific therapeutic drug monitoring and communicates relevant findings and/or recommendations to other health care providers in charge of the patient both orally and in writing.
- →Clinical Activities: Provides and documents continuity of care by extending refills (per policy) for patients without refills or refers patient to appropriate healthcare provider.
- →Demonstrates a working knowledge of VHA directives, VHA national formulary, Cincinnati VA policies and protocols including activity of the Pharmacy and Therapeutics (P&T) Committee to provide appropriate and cost-effective drug therapy recommendations.
- →Implements and monitors cost-effective drug regiments (protocols promoting positive patient outcomes).
