About the Role
The Clinical Pharmacist is a licensed pharmacist having completed a pharmacy residency or having other equivalent clinical experience. The incumbent is responsible for overseeing the daily general pharmacy services, clinical inpatient services and supporting the outpatient areas.

- →VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws The majority of the Clinical Pharmacist time will be spent providing comprehensive care to patients in the assigned practice area through clinical and distributive functions.
- →The incumbent will also assist physicians, residents, interns, nurse practitioners, and physician's assistants in active pharmacotherapy management of individual patients.
- →Also, the incumbent will be responsible for coordinating pharmacy services with nursing, patients and their families.
- →Duties include and are not limited to: Patient Care clinical activities: (1) Clinical Program Support: (a) Develops and maintains an in-depth knowledge in general and specialized areas of pharmacy practice appropriate to internal medicine, acute care and their area of practice.
- →(b) Develops professional relationships with non-pharmacy professional personnel, and projects the highest level of professional pharmacy practice.
- →(c) Provides management of patient therapy by utilization of advanced skills in physical assessment, pharmaceutics, pharmacokinetics, pharmacodynamics, and interpretation of laboratory and other diagnostic parameters.
- →(d) Reviews and evaluates prescribed drug therapy for appropriateness.
- →Assesses effectiveness, cost-benefit, adverse drug reactions, allergies, drug-drug and drug-disease interactions of patient's therapy.
- →Recommendations are communicated to providers and pharmacists and documented as necessary.
- →This includes evaluation and approval/disapproval of non-formulary requests as assigned.
- →(2) Distributive Program Support: (a) Review and verification of medication orders which includes correct and complete patient information, correct medication, dose, route, and frequency of administration in compliance with medical center medication use policies.
- →Orders are checked for patient allergies, drug interactions, incompatibilities, duplication, and priority.
- →(b) Identifies, interprets, and resolves prescribing and dispensing issues including pharmacotherapeutic and formulary issues.
- →Documents interventions as necessary.
