About the Role
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.
What You'll Do
- →The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) practices in Primary Care Service to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission.
- →The NP provides advanced clinical practice, consultation, and management.
- →The NP collaborates within a Patient Aligned Care Team (PACT) to provide clinical care to patients through an outpatient system.
- →The NP functions as the case manager for a complex Veteran population, providing hands-on and virtual patient care, and assumes primary medical responsibility for the patients assigned to their PACT team.
- →The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients.
- →Duties will include but are not limited to: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice.
- →Develops and implements an individualized plan to achieve optimal population health and disease management.
- →Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members.
- →Orders and interprets labs, tests, and diagnostic studies.
- →Provides timely follow-up on results, including completion of view alerts.
- →Performs and records history and physical examinations and records data from clinic visits.
- →Enters progress notes in computerized medical record and completes workload documentation in a timely manner.
- →Prescribes pharmacologic and non-pharmacologic interventions.
- →Initiates referrals and consults to specialty services.
Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓English Language Proficiency.
- ✓In accordance with 38 U.S.C. 7402(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.
- ✓Selected applicants will be required to complete an online onboarding process.
- ✓Acceptable form(s) of identification will be required to complete pre-employment requirements (https://www.uscis.gov/i-9-central/form-i-9-acceptable-documents).
- ✓Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
