About the Role
The Community Housing and Outreach Services (CHOS) Social Work Supervisor ensures for program integrity, overseeing the utilization of Housing and Urban Development - Veteran Affairs Supported Housing (HUD-VASH) Program vouchers in their coverage area within the VA Puget Sound. The Social Work Supervisor may also supervise staff from other CHOS programs, as assigned.
What You'll Do
- →Clinical Provides timely screening, psychosocial assessment, and diagnoses of mental disorders.
- →Develops, implements, and documents the psychosocial assessment, treatment plan, progress notes and updates, and discharge planning for provision of clinical services to Veterans.
- →Provides psychoeducation to Veterans and family members about mental health conditions and treatment methods.
- →Provides consultation to health care providers about assessment findings and treatment plans for Veterans.
- →Conducts: routine suicide risk screens; comprehensive suicide risk evaluations, and collaborative safety plans when clinically indicated; follows-up with Veterans who fail to attend appointments per local procedures; seek needed consultation with the Suicide Prevention Coordinator, Disruptive Behavior Committee, and/or CHOS Program Manager.
- →Provides consultation and supervision to social work trainees and unlicensed staff.
- →Provides leadership in identifying and addressing ethical issues that impact patients and staff.
- →Maintains complete patient confidentiality.
- →Analyzes ethical/care issues, applying appropriate theoretical principles and ensures application of institutional policy.
- →Provides considerate care, respecting patient/family and staff personal values and belief systems.
- →Participates in regularly scheduled clinical consultations with staff/treatment teams to ensure for quality and continuity of care, as well as individual Veteran attainment of program and treatment goals.
- →Administrative Provides training and supervision of a multidisciplinary team.
- →Provides administrative supervision to the team.
- →Monitors compliance of staff members with VA performance measures and effectively shapes provider practice patterns to improve standing on performance measures.
Requirements

- ✓Must be proficient in written and spoken English.
- ✓Subject to background/security investigation.

- ✓A valid state issued driver's license with a clean driving history in accordance with VA Policy to drive a government vehicle.
- ✓Ability to operate and drive a government vehicle as determined by an Occupational Health driving physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
