About the Role
The Social Worker provides comprehensive psychosocial services to Veterans receiving care through the Community Living Center (CLC) and Palliative Care Program. The incumbent conducts assessments, develops treatment plans, provides counseling and case management services, coordinates care with interdisciplinary teams, and connects Veterans and caregivers with appropriate VA and community resources.

- →Duties include but are not limited to: Conduct comprehensive psychosocial assessments and develop treatment plans for Veterans with complex medical, psychiatric, and social needs.
- →Provide case management, crisis intervention, counseling, and supportive services to Veterans, families, and caregivers.
- →Collaborate with interdisciplinary healthcare teams to coordinate treatment, discharge planning, and transitions of care.
- →Facilitate family meetings, advance care planning, and end-of-life discussions for Veterans receiving palliative and long-term care services.
- →Identify psychosocial barriers to treatment and coordinate referrals to VA, community, government, and supportive service programs.
- →Advocate for Veterans and caregivers by ensuring access to appropriate resources, benefits, and services.
- →Provide consultation to healthcare staff regarding psychosocial factors affecting patient care, treatment adherence, and quality of life.
- →Maintain timely and accurate clinical documentation in accordance with professional standards and VA requirements.
- →Participate in program improvement activities, team meetings, and other duties as assigned.
- →Work Schedule: Monday - Friday; 8:00 AM - 4:30 PM Recruitment Incentive (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized Pay: Competitive salary and regular salary increases.
- →When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
- →Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year).
- →Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
Requirements
