Join our Intensive Community Mental Health Recovery (ICMHR) team at the Salem VA Health Care System! This role delivers impactful services for Veterans through intensive case management, quality improvement, evaluations, psychosocial assessments, group and individual support, placement services, and coordination with community and hospital resources. What You'll Do

  • Develops a psychosocial assessment of the Veteran in collaboration with the interdisciplinary treatment team, the Veteran, family members, and significant others.
  • Responsible for contributing to the development of the per-centered recovery plan/treatment plan and setting achievable treatment goals with the Veteran and family members/caregiver and other clinical staff.
  • Serves as the subject matter expert on VA and/or community resources.
  • Refers the Veteran to needed services.
  • Acts as an advocate when it serves the best interest of the Veteran and family members/caregivers.
  • Makes rapid assessments and developing crisis management plans for admission to mental health inpatient programs.
  • Provides education related to VA and community resources, entitlements, Advance Directives/Living Will, and will refer Veterans and family members/caregiver to the appropriate healthcare professional for identified health education needs.
  • Reviews the progress notes from other providers to accurately determine the strengths and limitations of each Veteran for whom care is being provided.
  • Provides Veterans and their family members/caregiver with ongoing supportive counseling and social skills training.
  • Assists Veteran and their family members/caregiver in planning for discharge/aftercare by offering appropriate and viable discharge/aftercare options to the Veteran and appropriate family members/caregiver.
  • Responsible for transporting Veterans when said transportation is consistent with the Veteran's recovery plan (i.e., to address medical appointments, food insecurity, exploration of housing options, etc.).
  • In order to perform this function, incumbent is required to maintain a valid driver's license, routinely pass a driver's physical through Employee Occupational Health (EOH) at intervals determined by EOH and complete all required VA trainings related to driving a government vehicle.
  • Other clinical duties and responsibilities may be assigned by the supervisor outside of these primary duties that are the benefit of the Veteran, families, and quality of services provided.

  • Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
