About the Role

This Social Worker (Program Coordinator) position is in Physical Medicine and Rehab (PM&R) services at the VA Boston Healthcare Systems located at the Brockton location. This position is a full-time at 40 hours per week.

What You'll Do

  • The purpose of this position is to serve as the Polytrauma Program Coordinator Social Worker at the VA Boston Healthcare System.
  • The incumbent provides advanced clinical social work services while serving as the VISN 1 coordinator and subject matter expert for the Polytrauma System of Care.
  • Duties may include, but are not limited to: Serve as the VISN 1 Polytrauma Program Coordinator and clinical subject matter expert for veterans and service members with traumatic brain injury (TBI) and polytrauma.
  • Coordinate and oversee the delivery of comprehensive case management services across multiple VA medical centers and Community-Based Outpatient Clinics (CBOCs).
  • Conduct biopsychosocial assessments, develop treatment plans, coordinate interdisciplinary care, and evaluate patient progress to ensure appropriate rehabilitation and community reintegration.
  • Collaborate with physicians, nurses, psychologists, rehabilitation therapists, military treatment facilities, community partners, and other stakeholders to coordinate patient care and improve health outcomes.
  • Provide consultation, education, and technical guidance to interdisciplinary teams regarding polytrauma, traumatic brain injury, rehabilitation services, and available VA and community resources.
  • Lead multidisciplinary team meetings, facilitate case conferences, and coordinate communication among internal and external healthcare partners.
  • Develop, implement, monitor, and evaluate program initiatives, performance measures, quality improvement activities, and compliance with national, VISN, and facility policies.
  • Analyze program data, prepare reports, monitor outcomes, and recommend process improvements to enhance program effectiveness and patient care.
  • Coordinate outreach, education, and resource development activities for veterans, families, healthcare providers, and community organizations.
  • Mentor and provide clinical guidance to social workers and interdisciplinary staff regarding polytrauma case management and rehabilitation services.
  • Maintain advanced knowledge of VA benefits, rehabilitation services, military culture, and community resources to support veterans and their families.
  • Utilize electronic health records and VA information systems to document services, monitor patient outcomes, and maintain accurate clinical documentation.
