About the Role

The Purchased Non-Institutional Care (PNIC) Social Worker Program Coordinator is assigned to Home & Community Based Services (HCBS) Department in the Rehabilitation & Long-Term Care (RLTC) Division.

  • Educates medical center colleagues and/or students regarding the PNIC programs and assists with dissemination of PNIC information to other VA facilities.
  • Tracks PNIC statistics and identifies significant findings.
  • Also tracks spending and resource utilization and make decisions regarding resource allocation.
  • Identifies and develops program resources in the community, providing liaison with providers.
  • Also provides program oversight to ensure quality care is provided by contracted agencies.
  • Participates in research and research utilization.
  • Completes an initial standardized screening of Veterans to determine whether they meet eligibility criteria for inclusion in the PNIC programs.
  • Reassess Veterans on a regular basis to ensure they are receiving the appropriate level of services and remain in need of program support.
  • Develops a psychosocial treatment plan and sets achievable treatment goals with the Veteran and/or family in collaboration with PNIC team members.
  • Addresses the unique needs of homebound, frail, disabling, chronic, and/or dying Veteran.
  • Serves as a liaison between the PNIC provider and Veteran and/or family.
  • This includes initiating services, reducing or increasing authorized hours, addressing conflicts between the Veteran and/or family and agency/day center, and/or terminating services.
  • Educates the Veteran and/or caregiver of the available services and assists them in establishing the appropriate referrals based on the Veteran's preference or that of his surrogate decision-maker.
