About the Role

The incumbent in this position functions as HUD V ASH Program Coordinator for the Pacific Island Health Care System's HUD/VA Supportive Housing (HUD-VASH) Program. The HUD-VASH Program is at several sites throughout the Pacific Islands including Oahu, Kauai, and Guam. The incumbent is responsible for providing case management and other services for veterans receiving housing through HUD Section 8 Housing Choice Vouchers.

  • The incumbent actively participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedures or policy changes based upon work experiences.
  • The incumbent is responsible for following local policies for the HUD-VASH Program.
  • This includes following policies and documentation requirements as required for CARF and JC accreditation.
  • Takes responsibility for improving professional skills, participating in planning and executing specific staff development projects, attending seminars and conferences, and participating in training programs of other disciplines as requested.
  • The incumbent performs accountability functions: 1) documents all contacts with patients/families in the medical charts; 2) completes statistical and other reports in accordance with VA Medical Center and the Homeless Operations Management and Evaluation System (HOMES).
  • Works with VA Medical Center multi-disciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to HUD-VASH Program veterans.
  • Is responsible for providing care coordination for a caseload of veterans in the HUD-VASH program, assigned at a ratio of 1 case manager for approximately 25 to 35 vouchers.
  • The position involves driving to sites in the community and a valid driver's license is required.
  • Provides referral to VA medical and mental health inpatient and outpatient treatment and to other VA services including VA benefits, as well as community-based social services and other non-­VA entitlement programs.
  • Assists veterans in maintaining appointments at VA medical centers and other community agencies for medical, psychiatric, substance abuse, vocational and other services consistent with veteran's goals associated with healthful independent living.
  • Handles conflict and problems in dealing with the consumer constructively and appropriately.
  • Has working knowledge and skill in the use of word-processing, data management and other computer systems, especially those programs in use by VHA.
  • Assumes the role of team leader in the HUD-VASH Program team system.

  • Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).

  • Possess a valid, unrestricted State driver's license; Have a Safe Driving Record, and; Demonstrate that they are medically qualified to operate the appropriate motor vehicle safely.
