About the Role

This position is located at the Thomas H. Corey Veterans' Hospital in West Palm Beach, Florida. The Community Residential Care Program (CRC)/Veteran Directed Care (VDC) Program Coordinator is a Licensed Clinical Social Worker responsible for managing the daily operations of the CRC & VDC programs, as well as provide direct patient care. This position serves all Veterans enrolled in CRC and VDC programs within the catchment area of the West Palm Beach VA Healthcare System.

What You'll Do

  • This position serves all Veterans enrolled in CRC and VDC programs within the catchment area of the West Palm Beach VA Healthcare System.
  • Duties may include, but are not limited to: Oversee the development, recruitment, and function of residential homes and community-based care.
  • Case management and development of policies, care processes, and coordination with interdisciplinary treatment teams throughout the VA Medical Center.
  • Select residential homes and program beneficiaries, establish intake processes, oversight responsibilities, and maintain record-keeping systems.

  • Provide outreach, marketing, and training to promote the CRC and VDC models of care and mentor other VA program coordinators and healthcare providers.
  • Receive and complete consults from interdisciplinary teams on complex cases using advanced skills.
  • Conduct psychosocial assessments, interviews, and evaluations to establish facts about Veterans' situations and needs.
  • Collaborate with interdisciplinary teams to develop comprehensive assessments, highlighting strengths, limitations, and service needs to optimize functional status.
  • Develop psychosocial treatment plans with interdisciplinary teams, setting goals based on clinical assessments.
  • Make decisions regarding the use of VA and non-VA services and referrals, providing consultation and recommendations to the treatment team.
  • Establish ongoing relationships with caregivers, Veterans, and family members to evaluate progress towards goals and adjust treatment plans as necessary.
  • Follows protocol for abuse cases, medical or psychiatric emergencies, and keeps interdisciplinary teams informed.
  • Develop crisis management plans and provide rapid assessments to maintain Veterans in community residential care homes.
