About the Role
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
What You'll Do
- →Total Rewards of a Allied Health Professional The Care Coordination, Advocacy, Treatment, and Connections to Housing (CATCH) Social Work Program Coordinator provides professional, clinical, and administrative case management for the CATCH Program in the Social Work Service care line.
- →The (CATCH) Social Work Program Coordinator provides Social Work services on an outreach basis, specifically targeting Veterans experiencing homelessness who have high utilization of VANCHCS and community emergency departments.
- →Assists in developing individual or group goals and objectives as well as monitoring and coordinating the VANCHCS CATCH Program, follows VA-OIG, JCAHO and state standards in the performance of duties.
- →Collects and submits participant data into CPRS, REDCap, HOMES, and other VA auditing procedures on a regular basis.
- →The (CATCH) Social Work Program Coordinator independently performs case finding functions and outreach, identifying and screening patients for psychosocial needs.
- →Possess advanced knowledge of human development and behavior (physical and psychological) and the differential influences of the environment, society, and culture.
