About the Role
Social workers assigned to Health Care/Homeless Veterans (HCHV) provide psychosocial services and/or short-term case management to Veterans who are homeless/assist them in obtaining housing as expeditiously as possible. Once engaged through outreach, HCHV social workers connect Veterans to VA/community resources that provide prevention, supportive services and permanent housing. As a mobile worker, provides community care, emergency shelters, meal sites, homeless Veteran stand down events.

- →Once engaged through outreach, HCHV social workers connect Veterans to VA and community resources that provide prevention, supportive services and permanent housing.
- →Providing outreach and case management services to engage Veterans experiencing homelessness, including those who are unsheltered or residing in non-VA shelters, programs, and Veterans at risk of homelessness.
- →Providing outreach and limited case management to Veterans residing in Grant and Per-Diem and Contracted Emergency Shelter programs and assist in providing coverage to other HCHV staff as needed.
- →Covering the Housing Hub, offering same-day services and homeless program linkage to unscheduled Veterans.
- →Independently conducting interviews and assessments for the purpose of connection to VA and community-based services, resources and care.
- →Developing, documenting, and implementing treatment plans with Veterans to address those needs.
- →Possessing extensive knowledge of eligibility criteria for VA homeless programs, local housing resources and how to access the local homelessness system.
- →Assisting unhoused Veterans to enroll in VA healthcare and apply for local, state or federal benefits, to include social security or VA service connection.
- →Utilizing Housing First and other evidence-based practice models to promote Veteran engagement and self-efficacy.
- →Advocating on behalf of Veterans who may have encountered barriers to accessing shelter, housing or care.
- →Coordinating with health care providers in VA medical centers and the community to minimize barriers to outreach services, avoid duplication of services and improve access to care.

Requirements
- ✓You must be a U.S.
- ✓Citizen to apply for this job.
- ✓Selective Service Registration is required for males born after 12/31/1959.
