Social Worker (General Mental Health Case Manager)

About the Role
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
What You'll Do
- →Total Rewards of a Allied Health Professional This position is located at the VA Boston Healthcare system, Jamacia Plain campus.
- →The General Mental Health (GMH) Social Work Case Manager (SW CM) is responsible for providing practical assistance and emotional support to Veterans receiving Outpatient Mental Health (MH) care.
- →Care coordination, Treatment planning, and Case management: The social worker coordinates care within VA, and across VA/Community Services.
- →The social worker participates in interdisciplinary treatment team meetings and treatment planning.
- →S/he facilitates the development/updating of treatment, in collaboration with the Veteran/family and with interdisciplinary treatment team.
- →The social worker has working knowledge and experience in use of medical and mental health diagnoses, disabilities and treatment procedures; this includes acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology.
- →The social worker has basic knowledge of Veterans' benefits and services relating to special programs, service-connected compensation, and non-service-connected pensions.
- →The social worker facilitates referrals based upon Veterans' needs and eligibility.
- →The social worker serves as a liaison to community agencies and networks with agency representatives.
- →Education/Health promotion and prevention: The social worker provides education related to VA and community resources, entitlements, advance directives/living will and will refer Veterans/families to the appropriate interdisciplinary team member for identified health education needs.
- →Documentation: The social worker documents in the patient's medical record in accordance with Social Work, Medical Center policies, the Mental Health Service, and the requirements of Joint Commission.
- →This includes a statement of needed care and development of an integrated assessment and interdisciplinary treatment plan within the time frame required by Mental Health Service and Social Work Service standards.
- →Progress notes are timely and reflect progress toward treatment goals.
- →Outcomes are evaluated and documented.
