About the Role

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

What You'll Do

  • Total Rewards of a Allied Health Professional This Social Worker Position serves as a Behavioral Health Interdisciplinary Program (BHIP) Case Manager, independently providing expert clinical and case management services to Veterans served by the main campus Behavioral Health Interdisciplinary Program (BHIP).
  • BHIP provides treatment coordination and service delivery for Veterans who require higher level mental health specialty care than can be provided in the primary care setting.
  • BHIP teams are interprofessional and rely on the collective expertise of multiple disciplines to provide care for Veterans who have severe and/or complex mental health needs.
  • Duties include but are not limited to: Utilizes the Collaborative Chronic Care Model (CCM) to facilitate coordinated, proactive, population-based care.
  • Adheres to referral management procedures focusing on timely and person-centered care coordination, as well as continuity for Veterans.
  • Utilizes procedures to promote re-engagement, referral, or transition to other levels of care, as clinically determined based on patient's stability and recovery.
  • Conducts psychosocial assessments.
  • Develops treatment plans in collaboration with the Veteran/family and with the interdisciplinary treatment team.
  • Independently provides consultation services to other staff regarding the psychosocial needs of Veterans and the impact of psychosocial problems on health care and adherence to treatment plan.
  • Has working knowledge and experience in use of medical and mental health diagnoses, disabilities and treatment procedures including acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology.
  • Establishes and maintains effective therapeutic relationships with Veterans and their families.
  • Works with patients and families who are experiencing a variety of psychiatric, medical and social problems utilizing individual, and family counseling and therapy skills.
  • Work Schedule: Monday-Friday 8:00am - 4:30pm; subject to change based on the needs of the facility Recruitment Incentive (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized EDRP Authorized: Former EDRP participants ineligible to apply for incentive.

  • Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE).
