About the Role

The Program Coordinator is a member of the PACT team in Gillette, WY, providing direct services to Veterans & their families. This role delivers comprehensive case management support, including assessments, treatment planning, & coordination of psychosocial services. Core responsibilities include conducting outreach, collaborating with interdisciplinary team members, offering consultation & education to staff & community partners, & maintaining program data for planning & evaluation.

What You'll Do

  • Total Rewards of a Allied Health Professional Interviews Veterans & their family members/significant others to establish facts about the Veteran's situation, presenting problems & their causes, & the impact of such problems on the Veteran's functioning & health as part of a psychosocial assessment.
  • Uses subjective & objective data in making clinical assessment of the Veteran's needs.
  • Evaluates the Veteran's situation, including the Veteran's reaction & ability to deal with it, & arrives at a reasoned conclusion.
  • Assess serious & complication cases involving psychiatric illness, catastrophic medical conditions, dementia & other high-risk diagnosis.
  • Makes independent professional decisions & recommendations for treatment.
  • Finds suitable means of treatment to help Veterans and/or significant others cope with stressful situations.
  • Develops psychosocial treatment plans in coordination with interdisciplinary teams members, including goals for treatment.
  • Provides clinical services to Veteran's & family members/significant other in support of the Veteran's treatment.
  • Gives advice, guidance, emotional support, & other assistance.
  • Provides individual & group counseling services, individual & group counseling services, including psychotherapy in some setting.
  • Assisting Veterans with Digital Divide/Telehealth issues.
  • Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences & serves as liaison to family members.
  • Effectively assess for suicide risk providing clinical intervention to include: safety planning, referrals, education, & identification for need for higher level of care & referral.
  • Responsibilities include identifying psychosocial stressors that cause distress, impact health, or create barriers to care, such as adjustment to medical conditions, untreated mental health or substance use disorders, economic instability, legal concerns, & inadequate housing or transportation.
