About the Position: This is an open continuous announcement. Multiple vacancies exist and may be filled from this announcement at any of the locations listed. This is a Direct Hire Solicitation Salary negotiation may be available for those candidates who are new to Federal service. Non-Federal work experience may be creditable for leave purposes.
What You'll Do
- →Provide mental health initial screening and follow-up care for a high-risk population in a collaborative team approach.
- →Conduct initial patient interview to collect factual information and assess the needs of the patient.
- →Perform direct patient support through patient referrals and education.
- →Assist provider(s) in the development of criteria for standards of care and patient outcomes.
- →Operate computer to input data and monitor reports.
- →Maintain ongoing follow-up with patients identified in caseload.
