About the Role

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

What You'll Do

  • The incumbent is responsible for providing case management for a caseload of veterans in the HUD-VASH program.
  • HUD-VASH is an intensive case management program for chronically homeless veterans aimed at providing psychosocial services to veterans while residing in stable housing through HUD's Section 8 voucher system.
  • The incumbent performs site visits of HUD-VASH apartments to ensure that veterans reside in safe environments in compliance with local housing codes.
  • Incumbent performs specialized treatment of complex physical or mental illness.
  • Incumbent incorporates complex multiple causation in differential diagnosis and treatment of veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice.
  • Incumbent is able to provide individual, group, and family psychotherapy and advanced-level case management interventions used in the treatment of veterans with complex medical comorbidities, homelessness, post-traumatic stress disorder, addictions, and other serious disorders.
  • The incumbent will participate in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate.
  • The incumbent will act as a consultant to other team/staff members.
  • The incumbent will document social work interventions and activities in the patient's clinical record utilizing CPRS and ensure appropriate hand-off when transferring patient care.
