About the Role
The Outpatient Staff Registered Nurse (RN) is responsible for providing competent, evidence-based outpatient care to assigned patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting. The incumbent of this position will be working on the Patient Aligned Care Team (PACT) at Tomah VA Medical Center's Community-Based Outreach Clinic (CBOC) in Wisconsin Rapids.
What You'll Do
- →The Registered Nurse (RN) - Outpatient / Patient Aligned Care Team (PACT) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
- →The person in this role assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
- →The RN - Outpatient (PACT) is also responsible for the following duties: Assisting in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization consultation.
- →Demonstrating leadership in delivering and improving holistic, outpatient care through collaborative strategies with others.
- →Evaluating practices in an ongoing process, based on best evidence.
- →Providing peers with informal constructive feedback for improvement.
- →Fostering a safe and supportive environment conducive to the professional development of healthcare professionals.
- →Contributing professional nursing perspective in discussions with the interdisciplinary team.
- →Partnering with others to effect change and produce optimal outcomes.
- →Supporting colleagues through knowledge sharing to provide safe, quality outpatient nursing care.
- →Participating in activities and strategies to sustain evidence-based-practice.
- →Other duties, as assigned.

Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
