About the Role
THIS IS NOT A REMOTE POSITION The Registered Nurse (RN) - Home Telehealth (HT) Care Coordinator is responsible for providing competent, evidence-based care to assigned patients. The incumbent of this position works with potential non-VA community partners to improve access to rural and highly rural Veterans to healthcare services via telehealth initiatives and programming.
What You'll Do

- →TELEWORK IS NOT AUTHORIZED.*** The RN - HT Care Coordinator is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
- →The incumbent assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
- →Also, the HT Care Coordinator RN coordinates care for a panel of patients throughout the continuum of care to assure that care is timely, appropriate, of high quality and cost effective.
- →The RN - HT Care Coordinator is also responsible for the following duties: Facilitating shared decision making with the Veteran, identifying Veteran needs/goals and development/modification of a patient-specific plan of care.
- →Advocating for telehealth program needs at the facility, bringing matters requiring higher level intervention to the attention of facility leadership.
- →Facilitating communication across primary care, specialty care, other healthcare professionals and team members, other clinics, and community agencies.
- →Monitoring core quality and performance measures for all telehealth programs.
- →Developing mechanisms to evaluate and monitor the quality, safety, efficiency, utilization, productivity, and effectiveness of telehealth.
- →Other duties, as assigned.

Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.

- ✓Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
- ✓You may be required to serve a probationary period.
