About the Role

The Registered Nurse - Outpatient Patient Aligned Care Team demonstrates integration of biopsychosocial concepts, cognitive skills, and technically competent practice while providing care to clients. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.

What You'll Do

  • The Patient Aligned Care Team (PACT) Registered Nurse (RN) is responsible for providing competent, evidence-based gender-specific primary care to patients with oversight of licensed vocational nurses/nursing assistants as appropriate to the clinic setting.
  • This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department.
  • Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels.
  • Other duties included but not limited to the following: Manages nursing care of patients, organizes, and completes own assignments in a safe and timely manner, assisting team members as necessary.
  • Administers medications and procedures per established policies and guidelines.
  • Integrates patient preferences into daily delivery of quality patient care.
  • Demonstrates the ability to communicate confidently with members of the healthcare team and demonstrate professional behaviors that promotes cooperation and teamwork.
  • Influences care outcomes by collaborating with members of the interdisciplinary team.
  • Provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management.
  • Will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization.
  • Partners with the patient, family, and interdisciplinary team to promote effective and safe transitions across the health care continuum, including the Veteran's goals of care.
  • Fundamental primary care experience with the situations in which they are ultimately expected to perform, integrating theoretical concepts, basic critical thinking skills, and technically skilled practice.
  • Treats all patients, families and other caregivers with dignity and respect incorporating knowledge, values, and beliefs into nursing care delivery without judgement or discrimination.
  • Provides the Veteran's family, caregiver and/or significant other, the Veteran's representative, visitors to VA facilities, all VA staff, and other customers with consistent information according to establish policies and procedures.

Requirements

  • Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
  • Applicants who refuse to be tested will be denied employment with VA.
  • Selective Service Registration is required for males born after 12/31/1959.
  • Subject to background/security investigation.

  • Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
  • Must pass pre-employment physical examination.
  • Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
  • You may be required to serve a probationary period.
