About the Role
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (1 to 5 years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.
What You'll Do
- →The Home Based Primary Care-Outpatient Staff RN assesses, plans, implements, and evaluates care based on age-specific components.
- →Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
- →Administers medications and procedures per established policies and guidelines.
- →Influences care outcomes by collaborating with members of the interdisciplinary team. *Position requires travel fto patients homes within the Redding catchment area.
- →Major duties and responsibilities include, but are not limited to: Requires up to 75% travel for patient care, meetings, and training, drivier's license is required.
- →Responsible for providing competent, evidence-based care to Home Base Primary Care (HBPC) patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting.
- →Provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the HBPC Program.
- →Responsible for case management, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, and care planning.
- →Plans and delivers technically competent care in the home of Veterans with complex needs, including health promotion, health prevention, geriatric care, primary care, and palliative care.
- →Utilizes interventions, which maximize health capabilities and reduces unnecessary urgent care and emergency room visits, per Veteran's goals of care.
- →Makes home visits to Veterans in the program.
- →Assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization.
- →Demonstrates leadership in delivering and improving holistic, home-based care through collaborative strategies with others.
- →Evaluates practice in an ongoing process, based on best evidence.
Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.
- ✓Selected applicants will be required to complete an online onboarding process.
- ✓Acceptable form(s) of identification will be required to complete pre-employment requirements (https://www.uscis.gov/i-9-central/form-i-9-acceptable-documents).
- ✓Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
- ✓You may be required to serve a probationary period.
