About the Role
This Endoscopy Care Coordinator Registered Nurse within Gastrointestinal Specialty at Kansas City VA Medical Center, demonstrate proficiency in practice based on conscious and deliberate planning. Self-directed for managing complex client situations. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
What You'll Do
- →Duties include, but are not limited to: Integrates knowledge, skills, abilities, and judgment and is self-directed in goal setting for managing complex situations.
- →Validates understanding of the rationale for care processes and decisions.
- →Questions clinical practices for the purpose of providing evidence-based care.
- →Demonstrates the ability to cope with and manage competing priorities.
- →Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings.
- →Coordinates care with multiple providers across all levels and sites of care.
- →Coordination also includes assuring services interface within the Veterans Affairs (VA) and community programs to provide continuity of care to patients.
- →Makes sustained contributions to healthcare by actively participating in healthcare facility or service-level committees or councils, establishing active relationships and demonstrating leadership in improving care beyond the immediate practice setting.
- →Leads with emotional and social intelligence.
- →Demonstrates resilience in coping with challenging and stressful situations, to assist others in increasing their self-determination and competence beyond the immediate practice setting Assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs.
- →This includes, but is not limited to, acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses; combat stress, residuals of traumatic brain injury; community adjustment; addictions and other health problems.

Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
