About the Role

This position is located in the Medicine Service of the Maryland Healthcare System, Baltimore, MD. The primary purpose of this position is to implement the Primary Care system guidance as described below at the Veteran Affairs Maryland Healthcare System.

What You'll Do

  • Major duties: Update and maintain PCMM team and position settings, panel assignments, and preceptor links on a monthly cycle.
  • Validate and input direct clinical FTE for each PACT member (PCP and teamlet) at division and CBOC sites.
  • Record and verify the exact room numbers used by each clinical PACT member Calculate and monitor panel sizes using the PACT Model Capacity Perform deep-dive analyses using VSSC, Pyramid, ProClarity, PACT Compass to identify trends and outliers.
  • Model and refine attrition/assignment algorithms.
  • Coordinate mass reassignments of patients when team composition changes.
  • Prepare concise leadership briefings and reports to (facility, VISN, national) that summarize capacity, performance, risks, and recommended actions.
  • Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met.
  • Selection at a lower grade level does not guarantee promotion to the full performance level.
