This position is located at the VA Northern Arizona Healthcare System under the Office of Chief of Staff. Incumbent is the PCMM for the medical facility and satellite clinics at VA Northern Arizona Healthcare System. The purpose of this position is to perform Program Analyst PCMM duties related to panel assignments and reports, including but not limited to PCMM panel maintenance.
What You'll Do
- →Major duties include but aren't limited to: Responsible for maintaining the accuracy of the information in the PCMM Web database and responds to data updates reported in VSSC.
- →Responsible for establishing and managing PCMM, MHMM, HBPCMM team and position set up for all primary care, mental health, and home based primary care teams at Northern Arizona VA.
- →Creates or modifies team and position settings, assign provider user class based on verified credentials, link the associated clinic(s), set maximum panel sizes, program clinical alert notification levels, and establish preceptor to associate provider links.
- →Uses multiple assignment/reassignment tools to move large groups of primary care and /or mental health patients to different teams and positions, when changes become necessary.
- →Responsible for reviewing and managing dually assigned PCMM/MHMM patients in conjunction with the Traveling Vet Coordinator (TVC).
- →Completes comparative analysis of VSSC reports or data sets with local PCMM data to identify data variance and ensure data integrity.
- →Delegates PCMM Web user permissions for facility staff members.
- →Ensures facility PCMM standard operation procedures (SOP) or Directives are in place and current, meeting all local requirements and stakeholder concurrence to meet VHA PCMM goals and objectives.
- →Ensures that PCMM users are knowledgeable in PCMM functionality and utilization.
- →Ensures that PCDPC data is entered for each PACT teamlet member in PCMM and entered as a portion of a FTE employee and is kept current and accurate.
