About the Role
The PACT Licensed Vocational Nurse (LVN) provides patient-centered, team-based nursing care to Veterans within the Patient Aligned Care Team (PACT) model at a Santa Rosa South VA Outpatient Clinic. The LVN delivers high-quality, coordinated care focused on prevention, chronic disease management, health promotion, and continuity of care. The LVN serves as a key member of the interdisciplinary PACT team, supporting timely access to care, effective communication, and population health management.

- →Duties and responsibilities of the PACT LVN/LPN include: Perform vital signs, pain screening, health factors, and clinical reminders.
- →Conduct patient intake, rooming, and visit preparation.
- →Administer medications and immunizations per VA policy and scope of practice.
- →Perform wound care, injections, EKGs, point-of-care testing, and specimen collection.
- →Monitor and document chronic disease indicators (e.g., diabetes, hypertension).
- →Recognize and report changes in patient condition to RN/provider.
Requirements
