About the Role
About the Position: This position is with 436th Medical Group located at Dover Air Force Base, Delaware. This is a Direct Hire Solicitation A recruitment or relocation incentive may be authorized. Salary negotiation may be available for those candidates who are new to Federal service. Student Loan repayment may be authorized. Non-Federal work experience may be creditable for leave purposes.
What You'll Do
- →Facilitate continuity of care through a multidisciplinary approach in the treatment of depression, post-traumatic stress disorder (PTSD) and anxiety disorders in adult beneficiaries.
- →Provide patient education as an integral part of the services offered (e.g. medication education, alcohol effects on mood, etc.).
- →Communicate with providers regarding patient status and documents appropriately in the electronic medical record.
- →Coordinate patient referrals to support services provided through the internal and external community.
- →Assist clinic with patient and staff education and provide individual and small group training in order to increase referrals and promote understanding of the role of the mental health care facilitator.
