Demonstrates biopsychosocial concepts, cognitive skills, and technically competent practice while providing care to clients. Assesses, plans, implements, and evaluates care based on age-specific components. Responsible for the coordination of care focused on patient education, self-management, customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care by collaboration with the interdisciplinary team.
What You'll Do
- →The Registered Nurse (RN) Maternity Care Coordinator (MCC) coordinates and tracks maternity care for all pregnant Veterans throughout all aspects of the medical center, including Community Based Outpatients Clinics, as well as community vendors and/or agencies.
- →Functions as a liaison between the Veteran, contracted provider, and the local VA medical facility to ensure access to maternity care and appropriate delivery of service.
- →Educates Veterans on a variety of pregnancy-related topics, to include routine medical screenings and authorized VA and community-based resources for maternal, obstetrical, and infant care.
- →Contacts pregnant Veterans on a routine basis (minimally every trimester) throughout the duration of the pregnancy to assess clinical and/or social care needs, facilitating care and services between VA and non-VA care providers.
- →Screens all pregnant and postpartum women for depression using an appropriate screening tool and ensures that positive screening results are followed by further assessment, diagnosis and treatment if required.
- →Assesses for stability of the Veteran's home setting, finances, food security and support systems and coordinates with Social Work to facility access to local and community resources as needed.
- →Orders appropriate equipment from the Prosthetic Department for pregnant Veterans who have presented to VA emergency departments for pregnancy-related emergencies to secure appropriate follow up care such as lab, ultrasound, emotional/psychological support, and/or other care as needed.
- →Ensures seamless and timely re-entry into Primary Care.
- →Documents patient care interactions accurately, succinctly, and timely, using appropriate notes, templates, and clinical reminders, in the electronical medical record.
- →Maintains pregnancy-related tracking systems as required by VHA Directives and Handbooks.
- →Places an electronic alert to the Veteran's computerized health record indicating that she is pregnant or lactating and removing the alert once the pregnancy and lactation period has been completed.
- →Relocation/Recruitment Incentives: May be authorized for a highly qualified candidate Preferred Experience: Women's Health experience on Maternal-Newborn care experience.
- →National certification in Maternal Newborn-RNC-MNN, IBCLC, Ambulatory Care or Case Management.
- →Expert use of databases and tracking, involvement with Performance Improvement initiatives with outcomes.
Requirements
