About the Role

The Home Based Primary Care (HBPC) Outpatient Staff Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex HBPC care. This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the HBPC Program.

  • Portland is a port city in the Pacific Northwest and the most populous city in the U.S. state of Oregon.
  • Situated in the northwestern area of the state at the confluence of the Willamette and Columbia rivers.
  • Portland is known for having the amenities of a major city (such as an international airport, an efficient public transit system, major league sports teams and many museums and art galleries) and the charms of a small city (such as plentiful arts and crafts fairs, independent bookstores and local traditions like the annual Rose Festival Parade).
  • Rain or shine, you can take advantage of the city's proximity to nature by playing in the Willamette River, wandering through lush public parks and gardens, biking the city's 162 miles (261 km) of bike lanes and hiking trails both within city limits and in the nearby Columbia River Gorge.
  • Duties include, but are not limited to: Responsible for case management, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, and care planning.
  • Plans and delivers technically competent care in the home of Veterans with complex needs, including health promotion, health prevention, geriatric care, primary care, and palliative care.
  • Utilizes interventions, which maximize health capabilities and reduces unnecessary urgent care and emergency room visits, per Veteran's goals of care.
  • Makes home visits to Veterans in the program.
  • Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines.
  • Demonstrates performance and leadership that is broad enough to improve the care for the HBPC Program and facility.
  • Provides peers with formal or informal constructive feedback for improvement.
  • Supports colleagues and other nurses through HBPC knowledge sharing to provide safe, quality nursing care.
  • Evaluates outpatient outcomes of evidence-based decisions and practice changes for HBPC individuals, groups, and populations.
  • Responsible for the documented HBPC outcomes at the program or service level.

Requirements

  • Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
  • Applicants who refuse to be tested will be denied employment with VA.
  • Selective Service Registration is required for males born after 12/31/1959.
  • Subject to background/security investigation.

  • Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
  • Must pass pre-employment physical examination.
  • Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
  • You may be required to serve a probationary period.

  • Must have valid driver's license, proof of insurability, and be willing to drive
