About the Role
Provides case management services to individuals and communities, ensuring coordinated care, compliance with public health & clinical guidelines, & evaluation of outcomes. Develops care plans & serves as liaison to patients, providers, & care teams. This position is located at the Chinle Service Unit, Division of Nursing Services, & reports to the Director of Communicable Disease Prevention and Control. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).
