About the Role
About the Position: This position is located at the 23rd Medical Group at Moody AFB, GA. This is a Direct Hire Solicitation Management prefers license as Accredited Case Manager, Certified Case Manager, or Nursing Case Management. A recruitment or relocation incentive may be authorized. Salary negotiation may be available for those candidates who are new to Federal service. Student Loan repayment may be authorized. Non-Federal work experience may be creditable for leave purposes.
What You'll Do
- →Perform difficult and complex assessments.
- →Evaluate and analyze health care plan and resources.
- →Communicate with treating facilities, both in direct and purchased care to coordinate patient care and transitions.
- →Monitor, track and intervene as patients transition between levels of care.
- →Develop and recommend strategies to improve healthcare access, reduce administrative barriers for beneficiaries, and enhance the efficiency of the MTF.
