About the Position: This position is located at the 1st Special Operations Medical Group, Hurlburt Field Air Force Base, FL This is a Direct Hire Solicitation A recruitment or relocation incentive may be authorized. Salary negotiation may be available for those candidates who are new to Federal service. Student Loan repayment may be authorized. Non-Federal work experience may be creditable for leave purposes.
What You'll Do
- →Serve as expert mental health case manager to identify high risk and high-cost diagnosis.
- →Facilitate optimal patient outcomes by aligning treatment plans with best practices and the unique needs of each patient.
- →Monitor psychotropic medication use.
- →Communicate with treating facilities, both in direct and purchased care to coordinate patient care and transitions.
- →Chair and coordinate the weekly At-Risk/Multi-Disciplinary Treatment Planning (ARCT/MDTP) meeting.
- →Coordinate medical services needed to optimize current health, psycho-social, spiritual status, and benefits.
- →Conduct training, education, counseling, and clinical assistance in mental health case management techniques.
