About the Role
About the Position: This position is with the Defense Health Agency, Defense Health Network East at Martin Army Community Hospital located within the Department of Mental Health at Fort Benning, Georgia. This is a Direct Hire Solicitation
What You'll Do
- →Serve as expert mental health case manager and member of multidisciplinary team of medical and professionals to identify high risk and high-cost diagnosis.
- →Develop, implement, communicate and coordinate individualized empirically informed patient treatment plans that address all diagnosed conditions and health concerns.
- →Communicate with treatment facilities, both in direct and purchased care to coordinate patient care and transitions. -Communicate/coordinate with appropriate parties including command and other medical teams within the guidance provided by DHA.
- →Track client progress towards goals, modify care plans as needed, and advocate for clients' rights and preferences.
- →Coordinate meetings to ensure treatment plans for at-risk and high-interest patients are synchronized among all medical, Substance Use Disorder Clinical Care, Family Advocacy Program, and all other medical treatment providers.
- →Monitor psychotropic medication use, track physical and emotional side effects, recommend adjustments to therapy modalities, appropriate level of care placement, and provide clinical feedback to providers.
- →Schedule and monitor medical appointments, therapy sessions, and support services and ensure continuity of care across multiple systems.
- →Conduct regular resource and community needs assessments in order to be more informed about needs of client base.
