About the Role
About the Position: This position is located at Fort Riley, Kansas. See 'Additional Information' section for incentives offered. This is a Direct Hire Authority (DHA) solicitation utilizing the DHA for Certain Personnel of the DoD Workforce to recruit and appoint qualified candidates to positions in the competitive service.
What You'll Do
- →Serve as expert mental health case manager and member of multidisciplinary team of medical and professionals to identify high risk and high-cost diagnosis.
- →Perform difficult and complex assessments.
- →Facilitate optimal patient outcomes by aligning treatment plans with best practices and the unique needs of each patient.
- →Develop, implement, communicate, and coordinate individualized empirically informed patient treatment plans that address all diagnosed conditions and health concerns.
- →Ensure treatment plans for at-risk and high-interest patients are synchronized among all medical, Substance Use Disorder Clinical Care, Family Advocacy Program, and all other medical treatment providers.
- →Coordinate medical services needed to optimize current health, psycho-social, spiritual status, and benefits.
- →Develop and recommend strategies to improve healthcare access, reduce administrative barriers for beneficiaries, and enhance the efficiency of the Medical Treatment Facilities (MTF).
