What You'll Do

- →PACT, specialty care) including Non-VA Care providers, pharmacists, social workers, dieticians, behavioral health staff, etc. to provide a robust interdisciplinary approach to care.
- →The incumbent performs clerical and administrative support in the coordination, care, and treatment provided to patients throughout the Community Care (CC) Program referral processes in the Columbia VAMC.
- →The duties include but are not limited to: Schedules non-VA clinic appointment using Integrated Scheduling Solutions (ISS) and HealthShare Referral Management (HSRM)scheduling options.
- →Interprets outpatient Community Care and Mission Act policies to patients, their families, agency employees, and Veterans' organizations.
- →Collaborates with the Referral Coordination Initiative Team when a consult requires clinical guidance during the scheduling process.
- →Communicates information between community providers and VA interdisciplinary team timely and clearly.
- →Identifies and communicates patient concerns or needs recognized during communications regarding upcoming Community Care.
- →Ensures the integrity of various databases, and data used in the management of administrative and customer service within the department.
- →Reports backlogs, difficulties in meeting deadlines, or difficulty in securing information from community providers or the medical record.
- →Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
- →Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
- →Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.

Requirements
