About the Role
The Licensed Vocational Nurse (LVN) Spinal Cord Injury provides care to patients with the goal of promotion and/or maintenance of health and/or prevention of illness and injury based upon the nursing process which includes performing services requiring technical, manual, and basic assessment (data collection) skills. The LVN participates in evaluating, planning, and executing interventions in accordance with the concept of good nursing care and per the patient's treatment plan.
What You'll Do
- →The LVN's duties include but are not limited to; The LVN Spinal Cord Injury PACT LVN is responsible for providing direct patient care to and assisting with care coordination of Primary Care patients within SCI Patient Aligned Care Teams (PACTs), supporting services and clinics throughout the VA San Diego Healthcare System.
- →PACT clinics operate Monday through Friday, with variable hours to support Veteran care needs. a.
- →Gather additional data (face to face, via phone or video conferencing) related to patient needs for treatment determination. b.
- →Participates in PACT performance measure activities such as, but not limited to, non-complex 2-day post-discharge calls, clinical reminder compliance, aspects of chronic disease management. c.
- →Assists with coordinating care of patients, including patients consulted through Clinical Video Tele-Health visits. d.
- →Assure patients have pre-appointment requirements completed prior to PCP appointments. e.
- →Cover other SCI outpatient clinics as needed.
