About the Role
The Licensed Vocational Nurse (LVN) is a member of the nursing team who works under the supervision of a Registered Nurse and performs assignments requiring licensure as a Vocational/practical nurse. Care may be provided to patients in a variety of settings, i.e. Ambulatory Care including but not limited Primary Care, Specialty Care, Mental Health, Surgery, Special Procedures, Home and Community Based Care.
What You'll Do
- →Total Rewards of a Allied Health Professional The Licensed Vocational Nurse (LVN) performs assigned duties in a manner that promotes quality health care, outstanding customer service and the maintenance of an environment that is committed to serving the veteran.
- →Assists the RN by engaging and documenting nursing practice which incorporates knowledge and skills related to assessing client conditions to include but not limited to vital signs, patient assessment, documentation of pain assessment/evaluation in the medical record and accurate transcription, administration, and documentation of medication according to established procedures.
- →Performs, reports, and records assigned nursing care in Geri/PACT Clinics, including, but not limited to: Data collection and documentation of all interventions completed during patient visit.
- →Sending assessment data collected to the RN for review and action.
- →Administers and documents immunizations per protocol NATVCBHCS policies.
- →Clinical reminder completion.
- →Documents transfers in CPRS and warm handoff reports given to receiving facility.
- →Completes and documents Nursing Notes in CPRS, as applicable, in a timely manner and closes the Encounter Notes closed for patients not seen by a provider.
- →Notes will be closed by the end of tour of duty daily.
- →Areas to be addressed include: Encounters: Reasons for visit; diagnosis codes; Service Connections; Primary assignment; Service connected conditions; Diagnosis tab; Procedures, etc.
- →Completes pre-appointment telephone calls for scheduled patients.
- →Reviews Message Manager and assist Team to address/complete and document pending alerts communicated throughout tour of duty to facilitate care and address needs of the caller within 48 hours.
- →As assigned, assures twice daily documentation of temperature for medication refrigerator.
- →As assigned, daily check of clinic AED and other required daily monitoring logs.
