About the Role

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

What You'll Do

  • The LPMHC will demonstrate effectiveness in assessing for problems or concerns unique to patient population status as military veterans while independently, or in collaboration with the interdisciplinary treatment team, family members, and significant others, arriving at a reasoned diagnosis of patient problems and needs.
  • The goal of the assessment is to highlight the veteran's strengths, limitations, and internal/external supports and service needs in order to optimize the veteran's functional status.
  • The incumbent demonstrates an understanding of the range of treatment and skills needed for all adult patients, including geriatric patients, related to their physical limitations, psychological needs, and age-associated illnesses and conditions.
  • The incumbent must have an understanding of the problems of advancing age and respect for the diversification of the aging veteran population, being sensitive to human growth and development, age-appropriate needs and concerns of the geriatric veteran, and be able to provide current and advanced techniques to the treatment of this veteran population.
  • Duties include but not limited to: The PCMHI LPMHC uses population-based practice management strategies and maintains competences in the following core domains.
  • PC-MHI Clinical Practice: Conduct functional assessments, based on 30-minute appointments and will triage/refer to specialty care services as appropriate.
  • Applies principals of population-based care (defines role and identifies problems rapidly).
  • Majority of all appointments are less than 30 minutes.
  • Uses brief assessments routinely (e.g., PHQ-9, PCL, AUDIT C, etc.).
  • Initial consultation appointments are based on functional assessments and treatment focuses on functional outcomes.
  • Maintains daily open access slots.
  • Next available "scheduled" appointment is within two weeks .
  • Provide brief mental health and behavioral interventions (e.g., 1-5 sessions for broad range of concerns).
  • Documentation skills Writes clear concise notes, generally 1⁄2-1 page long, with explicit impression, recommendations and plan for PCP use Notes document curbside consultation results.
