About the Role
The LTC Charles S. Kettles VA Medical Center is seeking a Full-Time Licensed Practical Nurse for the Patient Aligned Care Teams (PACT) at the Flint VA Community Based Outpatient Clinic (CBOC), within Ambulatory Care Service. The tour for this position is: Monday-Friday 7:00am-3:30pm. EDRP eligible- please see Additional Information section.
What You'll Do
- →Total Rewards of a Allied Health Professional The LPN/LVN is a member of the larger Team.
- →The LPN/LVN requires a general review of their work by an RN for appropriateness and conformity with license, policies and procedures.
- →The primary purpose of the PACT Licensed Practical/License Vocational Nurse is to perform assigned duties in a manner that promotes quality health care, positive customer service and the maintenance of an environment committed to serving the veteran.
- →The LPN is the technical expert for performing, reporting, and recording assigned nursing care, including, but not limited to: Direct patient care.
- →Rooms patients and completes all clinic reminders, obtains vital signs and completes any treatments as ordered by primary care provider.
- →Phlebotomy outpatient blood draws LPN Clinics: Toenail Clinic, Immunization Clinic Quality Metrics- EQM/Population Health Metrics: Tracks panels metrics and implements strategies to manage fallouts via patient outreach or team collaboration.
- →EKG, lab draws and pre-processing, lab quality monitors, and point of care testing.
- →Data collection and documentation intervention of patient visit in applicable nursing note.
- →Based on the patient's goals, reinforces through health coaching and other methods, patient and family health education, to include self-management, prevention and wellness.
- →Assists in medication reconciliation.
- →Notifies the provider of changes Measures, records vital signs and enters them in CPRS.
- →Applies skin tests and measures and documents results.
- →Visual acuity tests Performs Monofilament exams Closes encounters daily Checks the Crash Cart and AED per facility policy Complete the Blood Glucose meter check daily, as applicable.

Requirements
