About the Role

Demonstrate proficiency in practice based on conscious and deliberate planning. Self-directed in goal setting for managing complex client situations. Assumes responsibility for the coordination of care focused on patient educations, self-management, and customer satisfaction throughout the continuum of care. Follows procedures per established policies and guidelines.

What You'll Do

  • Duties and responsibilities of the Interventional and Specialty Care Coordinator include, but are not limited to: Care Coordinator Registered Nurse (RN) focus is on individual growth and development in practice with increased levels of self-direction.
  • This nurse brings principles and theory with limited application of critical thinking skills.
  • Bridges the gap between clinical surgical teams and outpatients ensuring seamless care transitions across the pre-operative, perioperative, and post-operative stages.
  • Verify that all pathology, imaging, consents, screenings, and clearances are uploaded to the Electronic Health Record (EHR) prior to surgery.
  • Instruct patients and families on pre-surgical guidelines.
  • Standardizing workflows to optimize surgical outcomes and reduce hospital re-admissions.
  • Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings.
  • The Care Coordinator assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs.
  • The Care Coordinator addresses psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation with the interdisciplinary care team.
  • Analyzes and manages laboratory and radiology results and provides information to the providers.
  • Advocates for holistic care through patient/family education and provider interaction and implements and educational plan to meet the patient care needs.
  • The Care Coordinator will complete phone calls and secure messages within 24-48 hours and reviews alerts throughout clinic hours and resolve promptly.
  • The Care Coordinator will cross-train to provide support in various specialty clinics.
  • Responsible for participating within unit based and/or organizational level Shared Governance councils, utilizing clinical expertise, evidence-based practice, and knowledge to assist with finding solutions to problems or issues to improve the quality care provided to our Veterans.

Requirements

  • Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
  • Applicants who refuse to be tested will be denied employment with VA.

  • Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
