About the Role
Provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the HBPC Program. Responsible for case management, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, and care planning. This nurse plans and delivers technically competent care in the home of Veterans with complex needs, including health promotion, health prevention, geriatric care, primary care, and palliative care.
What You'll Do
- →The Home Based Primary Care (HBPC)Outpatient Staff Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex HBPC care.
- →Utilizes interventions, which maximize health capabilities and reduces unnecessary urgent care and emergency room visits, per Veteran's goals of care.
- →The nurse will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization.
- →Partners with the patient, family, and interdisciplinary team to promote effective and safe transitions across the health care continuum, including the Veteran's goals of care.
- →Delivers fundamental knowledge-based care to assigned clients while developing technical skills.
- →Assesses, plans, implements, and evaluates care based on age-specific components.
- →Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
- →Administers medications and procedures per established policies and guidelines.
- →Influences care outcomes by collaborating with members of the interdisciplinary team.
- →Catchment areas from Vero Beach to Boca Raton.
- →The duties include but are not limited too: Make home visits to Veterans in the program and assess, plan, implement, and evaluate care based on age specific components.
- →Assume responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
- →Influence care outcomes by collaborating with members of the interdisciplinary team.
- →Support colleagues and other nurses through HBPC knowledge sharing to provide safe, quality nursing care, and foster a safe and supportive environment conducive to the professional development of health care professionals.
Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.
- ✓Selected applicants will be required to complete an online onboarding process.
- ✓Acceptable form(s) of identification will be required to complete pre-employment requirements (https://www.uscis.gov/i-9-central/form-i-9-acceptable-documents).
- ✓Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
